Request for Attorney Facilitative and Information Gathering Conference/Joint Meeting

  1. INSTRUCTIONS:
    If either party is represented by an attorney or record, and if any matter is referred by the Court for a Facilitative and Information Gathering Conference/Joint Meeting, the attorney for either party may request an Attorney Facilitative and Information Gathering Conference/Joint Meeting. Upon filing of said request, the matter will be scheduled by the Friend of the Court for an Attorney Facilitative and Information Gathering Conference/Joint meeting in lieu of the originally scheduled Facilitative and Information Gathering Conference/Joint Meeting. The attorney requesting the Attorney Facilitative and Information Gathering Conference/Joint Meeting MUST complete this form with SEVEN (7) days of the date of notice for Facilitative and Information Gathering Conference/Joint Meeting. The form MUST be submitted to the Friend of the Court. Upon receipt, the Friend of the Court will schedule an Attorney Facilitative and Information Gathering Conference/Joint Meeting and provide notice to the parties and attorneys of record.
