2. INSTRUCTIONS:

   If either party is represented by an attorney or record, and if any matter is referred by the Court for a Facilitative and Information Gathering Conference/Joint Meeting, the attorney for either party may request an Attorney Facilitative and Information Gathering Conference/Joint Meeting. Upon filing of said request, the matter will be rescheduled by the Friend of the Court for an Attorney Facilitative and Information Gathering Conference/Joint meeting in lieu of the originally scheduled Facilitative and Information Gathering Conference/Joint Meeting. 

   The attorney requesting the Attorney Facilitative and Information Gathering Conference/Joint Meeting MUST:

   • Complete this form with SEVEN (7) days of the date of notice for Facilitative and Information Gathering Conference/Joint Meeting. 
   • The form must be submitted to the Friend of the Court. DO NOT FILE IN THE CLERK'S OFFICE.
     
   • Upon receipt, the Friend of the Court will will schedule an Attorney Facilitative and Information Gathering Conference/Joint Meeting and provide notice to the parties and attorneys of record.
   • ** IT IS THE RESPONSIBILITY OF THE ATTORNEY REQUESTING THE ATTORNEY FACILITATIVE AND INFORMATION GATHERING CONFERENCE/JOINT MEETING TO IMMEDIATELY NOTIFY OPPOSING COUNSEL OR OPPOSING PARTY IF UNREPRESENTED, THAT THIS REQUEST HAS BEEN FILED WITH THE FRIEND OF THE COURT AND THAT THE CONFERENCE WILL BE RESCHEDULED**
