- PC 571-Acceptance of Appointment Form (PDF) PC 571 Acceptance of Appointment must be completed by the proposed guardian or proposed conservator and filed with the Court at the time the Petition is filed. While the document states “I have been appointed guardian” or “I have been appointed conservator”, you are not appointed unless and until the Court actually appoints you through an Order. We have you complete this form prior to your appointment so it is ready and in the file on the date of the hearing. Please fill in the file number and the individual’s name. If Paragraph 3 applies, please check that box. If it does not apply, just drop down, fill in the date, sign, type or print your name, address, telephone number, and date of birth.

- PC 669-Proof of Restricted Account and Annual Verification of Funds On Deposit (Conservatorship of Minor) (PDF) Within 28 days of your appointment as conservator of the minor, you must have the financial institution, in which the minor's funds are placed, complete and execute Proof of Restricted Account and Annual Verification of Funds on Deposit (PC 669). In addition, each year when you file the Annual Account, you must have the financial institution complete and file a new Proof of Restricted Account and Annual Verification of Funds on Deposit (PC 669). The financial institution will also need to attach its statement to that document verifying the balance(s) in the account(s).
